Horizon Homes Inc. Crisis Services (Crisis Line, Mobile Crisis Services, South Central Crisis Center)

CRISIS LINE

To reach Horizon Homes Inc. Crisis Services, call 877-399-3040.

Serves Rice County and the counties of Blue Earth, Brown, Faribault, Freeborn, Le Sueur, Martin, Nicollet, Sibley, and Wantowan.

MOBILE CRISIS SERVICES

Mobile Crisis Services provides support for children, teens, and adults experiencing a mental health crisis. Support is offered 24 hours a day, 7 days a week. It can be completed via telehealth or in-person (at home, at school, or at any other community location the client chooses).

The service is 100% voluntary and includes a crisis assessment, risk assessment, and treatment plan focusing on:

  • Problem-solving and coping strategies
  • Local resources and information
  • Crisis prevention
  • Brief ongoing support through stabilization

Services are offered in English and in other languages through Language Line Solutions. Grant funding is available if the client’s insurance doesn’t cover the cost.

SOUTH CENTRAL CRISIS CENTER

South Central Crisis Center provides support for adults with mental health needs. It is a 16-bed, short-term crisis unit, and admissions are accepted 24 hours a day, 7 days a week. Stays range from a few hours to up to 10 days.

The service is 100% voluntary and includes:

  • Short-term problem-solving and counseling
  • Medication education and monitoring
  • Symptom management and relapse prevention
  • Family education and support
  • Stabilization after discharge
